1. We accept Master Card and Visa for your convenience.

2. We ask that "cell" phones be turned off when entering the office as a courtesy to others who are awaiting their appointments and to the office.

3. Appointments: Patients are seen in a timely fashion. If you are unable to keep your appointment, we ask that you please call the office to cancel or to reschedule. We consider this a courtesy to the other patients who could utilize this time slot.

1. How long will I have to wait to be seen?

We understand that your time is valuable and that appointments may run late. Remember, you too will be given the time necessary when you are seen.

2. What do I need to bring?

- Any and ALL insurance cards.

- A "photo" ID (ie: drivers license, military ID, etc...)

- If a co-payment is required by "your" insurance at the time of your visit, you will be responsible for this.
(we now accept Mastercard and Visa)

- Films : If diagnostic studies were performed at a facility, it may be necessary to bring them with you.

- A list of medications and medical background.

3. Is paperwork required?

- YES : a short form is required to be completed. Therefore, we ask that you arrive on time for your appointment so that you are able to complete these papers.
